私は、SeafariJapan & other ブランド(以下、会社)に対し、下記の日付に撮影された、自分(と、いる場合には下記に記されている自分の子ども)の画像、映像を会社の出版物(紙媒体、ネット媒体、動画媒体におけるマーケティング材料を含む)や、広告に使用することを許可します。 私は、会社に対して自分(と、いる場合は下記に記されている自分の子供)のプライバシーや守秘に関する事柄が上記の出版物に使われる写真や動画からある程度推測できる可能性があることを理解し、画像、動画の使用を許可します。更に、私は下記に記されている子どもの親／法定後見人であり、会社が彼(ら)の画像及び映像を使用することを子ども(達)に代わって許可します。 参加する事は任意であり、私及び子ども(達)は、会社が作成するあらゆる出版物や広告に使用する画像及び映像に対し、いかなる場合でも金銭的報酬は受けないことに同意します。また、使用される画像及び映像の権利は私及び子ども(達)にはない事を理解しています。 私は、会社(請負人、従業員、及び会社の出版物に関わる全ての人)に対し、私及び子ども(達)が参加する事に関して一切責任がない事に同意します。 私は、会社のアクティビティに参加する場合の保険保障は、保険会社の保険内容により限りがあり、それを超える請求をすることができないことを理解しています。
I hereby authorise SeafariJapan and other brands, hereafter referred to as “Company's,” to publish photographs or videos taken on the below signed date of myself and/or the minor child or children listed below, for use in the Company's print, online and video-based marketing materials, as well as other Company publications. I hereby release and hold harmless Company from any reasonable expectation of privacy or confidentiality for myself and for the minor child and children listed below associated with the images specified above. Further, I attest that I am the parent or legal guardian of the child or children listed below and that I have full authority to consent and authorise Company to use their likenesses. I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other Company publications. I acknowledge and agree that the publication of said photos or video confers no rights of ownership or royalties whatsoever. I hereby release the Company, its contractors, its employees, and any third parties involved in the creation or publication of Company publications, from liability for any claims by me or any third party in connection with my participation or the participation of the minor children listed below. I further acknowledge that insurance cover for the Company activities participated in are limited by the insurance agency coverage and any claim cannot be in excess of this limit.
